Apple cider vinegar (ACV) is one of the most popular natural health remedies, primarily studied for blood sugar control and weight loss. Evidence from small clinical trials suggests the liquid form (not pills) may modestly reduce post-meal blood sugar spikes and support weight loss when combined with a calorie-restricted diet. However, it carries real risks to tooth enamel, the esophagus, and in rare cases the liver and pancreas.

Overview

Apple cider vinegar is produced by crushing apples and adding bacteria and yeast, causing fermentation that converts sugars to ethanol and then to acetic acid. Bottled ACV typically contains 4-6% acetic acid along with malic, lactic, and citric acid [1]. During fermentation, a stringy mass called "mother" forms, containing small amounts of minerals and phenolic acids [2].

Forms and Bioavailability

  • Liquid ACV: The most studied and effective form for blood sugar effects
  • ACV pills/tablets: Not effective for blood sugar control. Acetic acid content varies widely (0.4% to 30%+) and most pills do not list content [1]
  • Filtered vs. unfiltered: Unfiltered contains "mother"; unclear if this affects therapeutic value

Pills providing 45-700 mg of acetic acid were not effective for glycemic control, while liquid forms providing 1,250-1,500 mg of acetic acid were effective [3].

Evidence for Benefits

Blood Sugar Control

Multiple small studies show liquid ACV consumed with a meal can reduce the post-meal blood sugar rise. In healthy adults, half a tablespoon with a high-glycemic meal reduced blood sugar increase at 1 hour by 54% [4]. In adults with insulin resistance, the same dose reduced the blood sugar increase by 64% and improved insulin sensitivity by 34% [5].

In type 2 diabetes, a study of 110 people found 1 tablespoon daily for 3 months reduced fasting blood sugar by 7.5%, HbA1c by 7.2%, and total cholesterol by 6% compared to placebo [6]. Another study found a modest 10 mg/dL decrease in fasting blood sugar over 2 months [7].

Weight Loss

A study of 155 obese adults found 1 tablespoon ACV twice daily for 3 months produced 4.2 lbs weight loss versus no loss with placebo [8]. A study of 39 overweight adults found 2 tablespoons daily for 1 month with calorie restriction led to 8.8 lbs loss versus 5 lbs with diet alone [9]. However, a large-effect study included in a recent meta-analysis was retracted for inappropriate statistical methods [10, 11].

Digestion and GERD

No studies demonstrate ACV improves digestion. A study found ACV did not reduce heartburn and some participants reported worsening symptoms [12].

Always dilute in water. Do not let liquid linger in mouth. Rinse mouth afterward to protect tooth enamel. ACV pills have no established effective dosage [3].

Safety and Side Effects

  • Tooth enamel erosion: ACV causes more demineralization than energy drinks or soda. Rinse with mouthwash afterward [14]
  • Gastroparesis: May worsen delayed stomach emptying in type 1 diabetes [15]
  • Pancreatitis: Rare cases reported [16]
  • Liver injury: Rare cases reported with long-term use [17]
  • Potassium depletion: Possible with very large chronic consumption [18]
  • Pill hazards: Tablets can lodge in throat. Pills with >20% acetic acid are dangerous [19]
  • Skin burns: Do not apply undiluted to skin or near eyes [1]

Drug Interactions

  • Diabetes medications/insulin: ACV may lower blood sugar, requiring dose adjustments [1]
  • Potassium-lowering medications: Excessive ACV may compound potassium loss [18]

Dietary Sources

Apple cider vinegar is a condiment used in salad dressings, marinades, and recipes. The acetic acid responsible for its effects is present in all vinegars. Store in a cool, dry place (no refrigeration needed); shelf life is 3-5 years [1].
